data, and Part Two deals with legal and financial information. A number of pages in Part
Two are marked "Confidential" because they may contain sensitive information once
you have completed them. To keep such information private and protected from identity
theft, we suggest that you remove these pages from the book. Alternatively, you
could make a copy of the page, complete and file it in a confidential location, and write
on the page in the book who has this important information, or where it is located.
Due to changing life circumstances (for example, changing medications), we suggest
you complete this book in pencil to enable you to update it easily.
You may find it useful to purchase a binder and make two copies of the pages that
